New Client Information
Initial Assessment
During the first few sessions, I will spend time gathering information and building an understanding of what is bringing you to my office. The next several sessions will be custom-designed to help you to solidify and expand your goals of change.
Length and Frequency of Sessions
The initial assessment appointment will be 60 minutes. All sessions following are 45 minutes. However, a longer session may be scheduled, if needed. When this occurs, sessions are prorated accordingly. Typically, sessions take place once per week, but this can vary based on what is most appropriate for the client.
Insurance
I do not participate in any insurance networks at this time. This allows me to make optimal treatment decisions regarding length and type of therapy without the limitations imposed by third party payers. I am committed to providing you with the best, most effective treatment available.
If you have health insurance, your insurance company may reimburse part of the cost of your visit. I am able to provide you with a “super bill” upon request. This form will include the proper code numbers for diagnostic category and type of service provided, as well as any additional necessary information. You may submit this to your insurance company for out-of-network benefits or save it for your financial records.
You are responsible for knowing the terms of your policy and for keeping track of the extent of your coverage. To check your out-of-network benefits, I recommend that you call your insurance company. Some helpful questions can include:

Means of Payment
Cash, check, or credit card are accepted. A credit card is required to be kept on file for billing purposes.
Cancellation/ No-Show
I require session cancellations be made at least 24 hours in advance. If less than 24-hours notice is given and because the appointment time is reserved for you, it is necessary to charge unless we agree that an emergency has occurred.
